Visitation request form

Our policy on visiting your family members/loved ones in hospital

Weekdays Weekends and national holidays
Visiting hours 14:00-17:00 (last slot is at 16:30)
Appointment time No restrictions within the visiting hours
Available slots 12 groups 6 groups
Duration of each visit Private room: no limitations
Common room (including double-bed room): max 30 minutes per visit
Who can receive family and friends’ visitations Patients who are currently free of any restriction and have already received permission to have visitors.
Who can come to visit the patient Those who have already agreed with the patient (2/3 visitors per slot)
*Visitors must be junior high school students or older.
Wards accepting reservation via website 4C, 5B, 6A, 6B, 7A, 7B, 8B, 9A, 9B, 10A, 10B, 11S (Private rooms)
When you arrive at the hospital Stop by the info desk in the main entrance. During nighttime and holidays, please go to the security office entrance.
How to prevent infections during visitations
  • Refrain from coming if you have symptoms such as fever, sore throat or cough
  • Wait 7 or more days before coming if you caught COVID19
  • Wait 5 or more days before coming if someone living with you caught COVID19
  • Check your body temperature and health condition, disinfect your hands, and wear a mask when you come to the hospital
  • Do not remove your mask and refrain from eating and drinking during the visit
Please read carefully
  • Patients who are admitted to 11S (special private rooms), 5C, 8A, ICU, CCU, HCU will consider permission for the visits on a case-by-case basis.
  • As we have limited slots to accommodate visitors, please refrain from visiting the patients on consecutive days. Please leave 4/5 days between each visit and kindly refrain from taking two or more slots on the same day.
Others
  • If the family is summoned by the doctor, or the patient needs spare clothes, drinks, etc., the appointment is not required (although visitation is not allowed in this last case).
  • We will be easing the restrictions for husbands attending their wives during delivery. Please ask our midwives at prenatal checkups for more details about visitations.
FAQ
  • I did not receive the e-mail
    The mail may have ended up in the spam due to settings problems. Please look for this address: @east.ntt.co.jp
  • The text is not displayed correctly
    E-mail services like Docomo, AU, Softbank etc., may have problems displaying text messages. Please use an e-mail address with a different domain to avoid these problems.

Please select the ward you intend to visit and enter the required items such as date and time. You will be able to book a visitation for the same day (for requests submitted before 8:00 am) and up to 9 days later.

Please show the appointment confirmation on your phone to the information desk (or security office entrance on festivities and weekends).
After booking, you will receive a confirmation email to the registered address. The staff at the entrance will double check the contents and give you a visitor pass.
