INFORMATION ON TELEPHONE PRESCRIPTION

“TELEPHONE PRESCRIPTION” is to respond over the phone or Fax to patients who regularly visit us with chronic
diseases taking medication continuously in line with the “Basic Policies for Novel Coronavirus Disease Control”
collected by the Ministry of Health, Labor and Welfare from the viewpoint of preventing the spread of the COVID-19.
Patients will get doctor’s consultation over the phone upon their request, based upon which, the prescription will be
sent from us to a dispensing pharmacy specified by each patient. The patient will get the medication at the dispensing pharmacy pursuant to its each process. Under our process, the eligible patients will be the patients who have regular outpatient appointments. You will get a call from us on the date of your outpatient appointment for doctor’s consultation. At your next visit to us, please make the payment of your medical fees for the telephone prescription.

Eligibility: Patients who have an appointment with us on a regular basis.
※Except for Every Saturday, Sunday, and National Holiday. Application within 3 days from the appointment date will not be accepted.  
※You may be asked to visit us for a face-to-face consultation subject to the doctor’s judgement.
※Telephone consultation may not be accepted subject to the content of the scheduled examination(s).

STEP1 APPLICATION BY E-MAIL

※Except for Every Saturday, Sunday, and National Holiday. Application within 3 days from the appointment date will not be accepted.  
※Telephone prescription will not be available for patients of Gynecology & Obstetrics.

●For application, please have following four items of information ready.

  1. Your phone number for the telephone prescription
  2. Your email address
  3. Our Re-Visit slip for your outpatient appointment
  4. Name of your desired dispensing pharmacy (including a branch name)/Telephone Number/Fax Number (Medical Record Handbook)

●Please make it sure to send the following information to us by e-mail.
Title of the e-mail: TELEPHONE PRESCRIPTION

①Outpatient appointment date
②Content of the Scheduled Examination(s) (Not required if none)
③Clinical Department
④Full Name of the Doctor who gave you the prescription
⑤Name of your desired dispensing pharmacy (including a branch name)
⑥Telephone number of your desired dispending pharmacy
⑦Fax number of your desired dispensing pharmacy

STEP2 TELEPHONE CONSULTATION

●Please wait for our call on your outpatient appointment day.

  1. Telephone consultation will be minimum required consultation for the telephone prescription.
  2. You may be requested to visit us for a face-to-face consultation subject to your condition at the telephone consultation.
  3. If you will not be available when we call you, another phone call from us may not be made.

STEP3 RECEIVING MEDICATION

●The medication to be prescribed will be your usual medication, in principle.

●Please wait for contact from a dispensing pharmacy which you desired for.

  • The prescription will be sent from us to each dispensing pharmacy by Fax. Please note, however, it may take sometime for us to make the arrangement.
  • For details, please ask your dispensing pharmacy.

STEP4 PAYMENT OF YOUR MEDICAL FEES

●Please pay your medical fees for the telephone prescription at your next revisit to us.
