The main duties of the office are as follows. <\/li>\n<li>Although our hospital introduced an electronic medical record system in December 2000, paper medical files created prior to that time\u2014along with X-ray films, electrocardiograms, electroencephalograms, and certain forms that must still be retained in paper format\u2014are centrally stored to ensure that they can be accessed at any time when required for patient care.<\/li>\n<li>With regard to electronic medical records, the office monitors the status of recorded entries and the completion of discharge summaries and encourages physicians to ensure the accuracy and completeness of their documentation and to finalize summaries within the designated time limit.<\/li>\n<\/ul>\n<h2>How to Request Your Medical Information (Facility meets standards for medical record management)<\/h2>\n<p>At our hospital, doctors provide medical information as part of routine care to help build trust between patients and medical staff and to support high-quality treatment. If you would like more detailed information than what is provided during your care, you can submit a request for disclosure.
